All of our Hallmark Health joint patients are required to attend Joint Camp. It is an opportunity for you to establish a relationship with the team who will be responsible to you for all of your care during your joint replacement experience. It is well coordinated with your surgeon.
Features of Joint Camp include:
- A Joint Care Coordinator who will coordinate all or your preoperative, hospitalization, rehabilitation and discharge planning.
- Your choice of a family member or friend to become your “Joint Coach” so that they can participate in your care and recovery process.
- A personalized “Joint Journal” as a comprehensive guide for you to follow throughout your joint replacement experience.
- A comprehensive Preoperative assessment with:
- Nursing
- Anesthesia
- Physical Therapy
- Occupational Therapy
- Your Primary Care Physician if necessary
- A home environment assessment to evaluate your needs at home
- An equipment need assessment
- Assurance that you will have a private room post op
- A tour of the unit where you will be having surgery
- An opportunity to network with other Joint Camp participants
- A checklist to complete prior to your surgery
- We will provide any necessary preoperative testing
- A discussion of your preoperative exercise plan
You will only need to attend one Joint Camp session. Members of the team will be available for both you and your “coach”.
