At Hallmark Health, we provide Cardiac Rehabilitation to individuals who have experienced a heart attack, cardiac surgery, balloon angioplasty, stent placement, or those with stable angina pectoris or coronary artery disease.
The Cardiac Care Team has designed a comprehensive program from evaluation to exit that will help patients achieve and maintain a healthy lifestyle, including a personal exercise regimen.

The Lawrence Memorial Hospital and Melrose-Wakefield Hospital Cardiac
Rehabilitation programs consist of three distinct phases. Phase 1 begins while you are in the hospital. Phases 2 and 3 take place following your discharge from the hospital. Your cardiac physician closely monitors your care throughout all phases of the program.
While in the hospital, you begin physical therapy as needed and begin education on cardiac disease.
You will join a group of other patients to exercise while supervised by a cardiac nurse and exercise physiologist or physical therapist. A telemetry unit monitors your heart's performance during exercise. You will also attend group education classes covering a variety of cardiac care topics.
For our patients remaining at risk following completion of Phase 2, we offer continued supervised group exercise sessions 1 to 3 times per week. Once you have achieved a level of cardiovascular conditioning that will allow you to exercise independently, you are discharged from Phase 3.
To join the Cardiac Rehabilitation program, you must be referred by your physician. We accept patients from all physicians in the area. Patients who have been treated or had surgery at other hospitals are also welcome to participate in our program.
Research has shown that patients who participate in Cardiac Rehabilitation programs, including education and exercise, show improved functioning of the heart and lungs, improved management of risk factors, increased confidence and an enriched sense of well-being.

Our cardiac team is dedicated to optimizing your level of activity and function in a SAFE environment. Team members will continuously monitor your progress and modify your treatment plan accordingly
Health insurance coverage varies between insurance carriers. Some insurers require prior approval before enrolling in a Cardiac Rehabilitation program. Please consult your insurance provider concerning coverage prior to your first visit with us. Also, please be aware that copayments are required at time of visit and Phase 3 maintenance is not covered by any insurance carrier.
