This is a brief overview of benefits offered to eligible employees of Hallmark Health. These benefits are subject to change and/or elimination. In most cases, a more complete description of these benefits is available in the Human Resources Department. The official documents will govern in the case of any conflict or discrepancies. Hallmark Health is pleased to offer a cafeteria-style flexible benefits program. Hallmark Health provides a certain number of credits to equal the cost of one times salary for life and one of the options for long term disability insurance. In addition, the cost for health and dental insurance are subsidized by Hallmark Health. Most of the cost you pay for benefits through payroll deduction are not considered as taxable income by the IRS.
Employees may choose from three medical plan options including a Health Maintenance Organization (HMO), a Point of Service (POS) plan, and a Preferred Provider Organization (PPO) plan.
Employees may select from two options; one is a standard program, the other providing orthodontic coverage for all employees and dependents.
The vision care plan helps pay for vision-related expenses including annual eye exams, and the cost of lenses and frames, or contact lenses. There is a higher level of benefits if using network providers.
Employees may select from four different life insurance levels. In addition, there is an equivalent amount of accidental death & dismemberment (AD&D) insurance.
Allows employees to set aside pre-tax dollars for medical expenses or dependent care expenses.
Employees have the option of enrolling in three different plan options that help replace income during
disabilities starting as early as 7 days and continuing for up to 25 weeks.
Hallmark Health offers three levels of long-term disability after an elimination period of 180 days.
Hallmark Health offers both spousal and child life insurance.
Hallmark Health is pleased to offer a 401(k) plan with several investment options available. Employees may contribute 1 - 50% of their salary, and are eligible for a hospital match of 50% on the first 6% of compensation contributed as 401(k) pre-tax contributions, subject to the limitations set forth by law.
Eligible employees working at least 22 1/2 hours per week will receive Paid Time Off (PTO) benefits ranging from 25 - 40 days per year for full-time, pro-rated for part-time employees. Hallmark Health also offers a cash-out feature of 2 weeks of PTO time each year, as long as employees have accrued at least 1x their annual bank. PTO days are granted based on the position grade and length of service. In addition, there is a separate Extended Illness Bank (EIB) that grants up to 5 days per year to full-time employees (pro-rated for
part-time) to be used for medical leaves of absence of more than 3 days.
Benefit eligible employees may apply for tuition reimbursement benefits up to $4,000 per calendar year.
Hallmark Health will reimburse benefits-eligible employees for attending education and professional development programs up to $400 per calendar year for up to two days off, subject to the program guidelines.
A confidential counseling and referral service is available to employees and their families.
Hallmark Health has a weekly payroll and offers at no cost direct deposit to area banks.
Employees have the opportunity to purchase automobile, homeowners, and renter's insurance via payroll deduction.
Movie tickets, amusement park, and other attractions are offered at a discount to employees.
From time-to-time, Hallmark Health offers a variety of seminars on subjects such as estate planning, financial planning, etc.
